This form is for foreign employees (Employment Pass, S Pass, Work Permit, Letter of Consent, In-Principle Approval, Dependant’s Pass or Long-Term Visit Pass holder) who meet this criterion:

• Been to mainland China within the last 14 days upon arrival in Singapore on or after 31 Jan 2020 6pm.

This excludes foreign employees with Hubei-issued passports or recent travel history to Hubei who will be quarantined on arrival.

Notes for employer:

• Please arrange for affected employees on Leave of Absence (LOA) to sign the consent Form.
• Please retain this form and submit it to MOM when requested by the Controller of Work Passes.
