What is the Information or Document Required for Cancellation of Work Permit?

Request to cancel your worker’s Work Permit: Information needed: You will need these details for cancellation of work permit at WPOL:

• • Work Permit number

• • Date of application

• Worker’s departure details if he/she is leaving or has left Singapore (flight details (if any), departure date, and immigration checkpoint)

Cancellation as the worker is medically unfit, missing, in prison/remand, under investigation or has passed away: You need the following additional information or documents on hand for cancellation under these scenarios. Worker is medically unfit due to TB, VD or HIV:

• • Clinic’s name, address and contact number

• • Doctor’s name

• Date of the medical report

b. Non-Malaysian worker who is missing:

• • Missing person police report

• • Date the worker went missing

• Police report number and date

c. Worker has passed away:

• • Death certificate

• • Airway bill, cremation or burial certificate

• Cause of death

d. Worker is in prison/remand or under investigation:

• • A letter from the Prison confirming that the worker is in prison/remand or;

• • A letter from the authority confirming the investigation or;

• The investigating officer’s contact details
